When the documented process and the real process are different

A recent HSE prosecution involving an unguarded conveyor gives process manufacturers a reason to look closely at their own checks: would they pick up when the plant no longer matches the process described on paper?
A company was fined £160,000 after maintenance engineer suffered severe, life-changing injuries when his arm was drawn into a conveyor at an Industrial Park site.
HSE found that a hole had been cut into the side of the conveyor years earlier so employees could clear blockages while it was still running. A second conveyor was later found with a similar unauthorised access point.
The company had safe systems requiring machinery to be guarded and isolated before maintenance or cleaning. HSE also reported that the modifications had not been identified or rectified despite routine inspections, management walkarounds and audits.
For manufacturers, the issue is whether written procedures still reflect what people are actually working with on the plant floor.
Plant changes are not always obvious
Machinery rarely stays exactly as it was on the day it was installed. Parts are replaced, equipment is repaired and operators get used to the areas that regularly jam, trip or cause problems.
Some changes are planned, documented and signed off properly. Others happen more informally. A small adjustment may be made to keep production moving, or a temporary fix may remain in place because it appears to solve the immediate problem.
That is where the risk can start to grow.
Once a workaround becomes familiar, it may stop looking unusual to the people who see it every day. The equipment on the plant floor can gradually move away from the version described in procedures, risk assessments and inspection forms without anyone deliberately deciding to make that change.
For manufacturers, that makes change control about more than major engineering projects. It also means having a way to pick up the smaller changes that creep into normal working practice before they become accepted as part of the job.
Repeated faults can reveal more than the fault itself
A one-off blockage may be just that. When the same machine keeps stopping in the same place, the more important question is what happens next.
If operators are regularly dealing with the same fault, they will usually develop a routine for getting production moving again. That routine may follow the written procedure exactly, but over time it can also drift into something quicker or easier.
That is why recurring maintenance issues deserve more than a record showing that the fault was fixed. They can be a useful prompt to look at how the equipment is actually being used and whether the original controls still make sense.
Speaking to the people who operate and maintain the machinery is often an important part of that review. They are usually the first to know which problems keep coming back, where shortcuts are being taken and where day-to-day practice no longer matches the procedure.
That information then needs to reach the people responsible for maintenance, inspection and risk so the underlying issue can be properly reviewed rather than repeatedly dealt with as an isolated fault.
A completed inspection is not the same as an effective one
One of the easiest things to measure is whether an inspection has been completed. In a digital system, managers can quickly see what is due, what has been signed off and which actions are still outstanding.
That visibility is useful, but it can also create a false sense of reassurance if completion becomes the main measure of whether the process is working.
An inspection can be completed on time, every time, and still fail to identify a change that was never anticipated when the checklist was written.
A question such as “Is the guard fitted?” checks the condition the business expects to find. It may not prompt someone to notice that the machinery itself has been altered, or that a workaround has gradually become accepted practice.
That is why inspections also need room for judgement. Asking what has changed since the last inspection, what looks different, or whether the equipment is still being used as intended gives inspectors a better chance of spotting something that falls outside the original checklist.
The important figure is not simply how many inspections were completed. It is whether those inspections are still capable of finding the problems that matter.

When the plant changes but the paperwork doesn’t
Photographs, repeat observations, maintenance issues and previous actions can help show how equipment and processes change.

HSE does not explain why the earlier inspections failed to identify the modifications, so this is not about blaming individual inspectors.
